What is Close Sinus Lifting?

Closed sinus lifting (transcrestal or osteotome technique) is a minimally invasive procedure applied in patients with moderate bone insufficiency in the posterior upper jaw region. The sinus membrane is elevated through the implant socket and a small amount of graft is added.

This technique is preferred when existing bone height is between 5-8 mm and 2-4 mm bone gain is needed. Its greatest advantage is that it can be applied in the same session as implant placement and is less invasive than the open technique.

Treatment Stages

Imaging and Planning

Existing bone height and sinus anatomy are evaluated with 3D tomography. Suitability for closed technique is determined.

Implant Socket Preparation

Drilling is stopped 1 mm short of the sinus floor. Bone floor is preserved.

Elevation with Osteotome

Sinus floor is pushed upward without breaking using special osteotomes. Membrane is elevated.

Graft Addition

Small amount of bone graft is placed in the created space.

Implant Placement

Implant is placed in the same session. Primary stability is achieved.

Close Sinus Lifting Types

Summers Osteotome Technique

Sinus floor elevation with classic osteotome set. Widely used standard method.

Hydraulic Technique

Membrane elevation with fluid pressure. Provides controlled and safe lifting.

Balloon Catheter Technique

Membrane lifting with inflatable balloon. Minimizes perforation risk.

Osseodensification

Bone densification and sinus elevation with special burs. Improves bone quality at the same time.

How Is It Done?

Closed sinus lifting is a procedure integrated with implant surgery and is completed in 30-45 minutes under local anesthesia.

Local anesthesia is applied and implant site is prepared
Pilot socket is drilled until 1 mm short of sinus floor
Sinus floor is carefully pushed upward with osteotomes
Bone graft is prepared and placed on sinus floor
Implant is placed and primary stability is checked

Advantages

Minimally invasive - less surgical trauma
Can be applied in the same session as implant
Fast healing process
Less post-operative swelling and pain
Short surgical time (30-45 minutes)
Lower cost compared to open technique
Outpatient treatment - no hospitalization required
Low complication rate

Disadvantages

Limited bone gain (2-4 mm)
Insufficient for severe bone loss
Blind working - no direct vision
Requires experience

Note: Most of these disadvantages can be minimized with proper planning and treatment by an experienced dentist.
